For years, Ely Bair dealt with migraines, jaw pain and high blood pressure until a dentist recommended surgery to realign his jaw to get to the root of his health problems.

The solution would involve two surgeries over a couple of years and wearing braces on your teeth before and between procedures.

Bair had the first surgery on the upper jaw in 2018 at the Swedish Medical Center, First Hill Campus in Seattle. The operation was covered by his Premera Blue Cross plan, and Bair’s hospital costs were $ 3,000.

In 2019 he changed jobs, but was still insured with Premera. In 2020 he had the planned lower jaw surgery in the same hospital where he was treated for the first time. The operation went well, and he spent one night in the hospital before he was released. He healed well and began to see the benefits of the operations.

Then came the bill.

– The patient: Ely Bair, 35, a quality assurance analyst. He has a Premera Blue Cross health plan through his job at a biotech company in Seattle.

– Total Bill: The Swedish Medical Center billed Bair $ 27,119 for the second surgery in July 2020. This was Bear’s share of the negotiated rate after the hospital deducted $ 14,310 from the fee. His insurer paid $ 5,000. Bair owed the surgeon and anesthetist additional bills.

– Service Provider: Swedish First Hill Campus in Seattle, part of the largest non-profit healthcare system in the Seattle area connected to Providence, a large Catholic healthcare network.

– What gives: Bair encountered two insane health system pitfalls here: he expected his new plan to behave like his previous one from the same insurer – and he expected his mouth to be treated like the rest of his body. None of the common sense concepts seem to be true in the American healthcare system.

Typically, large corporations like Bair’s employers take out self-insurance, which means they pay for their employees’ healthcare bills, but use insurance companies to maintain provider networks and handle claims. When Bair switched jobs, his insurance coverage changed, even though both employers were using Premera. Bair paid $ 3,000 for his first surgery because that was the maximum out of his own pocket that he had from his previous employer who covered oral and maxillofacial surgery.

Bair assumed that the cost of the second part of his treatment would be similar if he used the same hospital and insurance carrier. Bair’s oral and maxillofacial surgeon – the same doctor who performed the first procedure – checked Bair’s benefits through his insurer’s online portal and thought they were covered. Premera also sent his doctor’s confirmation that the second procedure was medically necessary.

About three months after the operation, Bair was shocked to receive the huge hospital bill – about $ 24,000 more than he expected.

When he called Premera, he learned that his new plan has a lifetime coverage limit of $ 5,000 for reconstructive jaw surgery known as orthognathic surgery, which is sometimes viewed as dental rather than medical. His doctor said no information was recorded on Bair’s services when the practice reviewed them through an online portal. Premera told Bair he should have known about the limit because it was listed in his detailed, printed 86-page member benefits brochure.

The Affordable Care Act of 2014 removed lifetime and annual coverage caps for treatment categories such as prescription drugs, laboratory services, and mental health care. While the ACA lists broad categories of what qualifies as “essential health benefits”, each state decides which services fall into each category and what scope or duration must be provided. Bariatric surgery, physical therapy, and abortion are examples of treatments for which coverage under this ACA provision can vary widely by state. Orthognathic surgery is not seen as a major health benefit in Washington. It is sometimes done for cosmetic purposes only. In plans, surgery is also sometimes considered part of orthodontics – which is often associated with limitations in coverage. But for Bair it was a clear medical need.

Without an ACA orthognathic surgery requirement, premera and self-insured plans may offer different benefit levels and may impose annual and lifetime caps.

Premera spokeswoman Courtney Wallace said Bair had moved from a plan with his former company that didn’t have a lifetime maximum to a plan with a maximum lifetime benefit of $ 5,000.

Martine Brousse, a patient advocate and owner of AdvimedPro, who helps patients with healthcare billing disputes, said Bair acted appropriately by visiting a doctor and hospital on his health plan network and asking his doctor about his insurance coverage.

She said Swedish should have told him how much he would have to pay before the operation – which was scheduled weeks in advance. “This is a failure of the hospital,” she said.

Sabrina Corlette, co-director of the Georgetown University Center on Health Insurance Reforms, said it wasn’t fair that his first employer paid for his surgery but the second employer didn’t. She said the $ 27,000 bill seems excessive and the lifetime limit of $ 5,000 is very low. “Essential health benefits have a really important role to play, and when there are gaps or holes, people can really get hurt,” she said.

– Solution: Bair’s doctor told him that the hospital costs were at least three times the Swedish costs for uninsured patients for the same operation. Bair said Swedish offered to pay the bill over two years but made no other concessions.

Swedish wouldn’t say why it didn’t check Bair’s insurance benefits before surgery or let him know he was facing a huge bill even though he was insured.

“Hospital prices are complex and nuanced,” Swedish officials said in a statement. Bair’s bill “covered all of the care he received, including specialized services and skills, equipment, and operating room time. He had jaw surgery that had a maximum benefit from his insurer of $ 5,000. He was charged the balance that was not covered by his insurance. “

The hospital system said it also has an online tool that generates estimates tailored to patient care and hospital choice.

However, the online tool did not bring anything to the term “orthodontic surgery”.

Bair appealed three times to Premera to reconsider his decision to pay only $ 5,000 of the cost of his trial. But the insurer refused everyone, saying he had exhausted his lifetime benefit for orthognathic surgery and he was responsible for any additional treatment. When Swedish did not want to cut its costs, it filed a complaint with the prosecutor in December 2020.

A few months later, Swedish Bairs reduced the bill from over $ 27,000 to $ 7,164.

“Neither the patient nor his doctor knew about this limitation of coverage before the procedure, the surgeon has worked on behalf of the patient to lower the bill,” said the hospital to KHN in a statement.

Bair agreed to pay the lower amount. “The bill is at least a much more manageable number than the financial ruin of $ 27,000 would have been,” he said. “I’m just looking forward to closing this chapter and moving on.”

His surgeon, who helped him battle the hospital bill and limited insurance coverage, reduced his bill from $ 10,000 to $ 5,000, Bair said.

Bair said his employer, Adaptive Biotechnologies, is considering lifting his $ 5,000 lifetime limit on the procedure if medically necessary.

Since the operation, Bair said he had much fewer migraines and that his high blood pressure had dropped. “I feel a lot more energetic,” he said.

– The takeaway: In the event of a planned operation, talk to your hospital, doctor and insurer about the amount of the bill you are responsible for – and have it submitted in writing before each procedure.

“In theory, you should be able to rely on your provider to confirm your coverage, but in practice it is in your best interest to call your insurer yourself,” said Corlette.

Although the ACA has removed lifelong and annual coverage caps, this only applies to services that are deemed essential in a patient’s condition. Be aware that certain surgeries – such as jaw surgeries – are in a gray area; Insurers may not consider it a necessary medical procedure or a medical procedure at all. Corlette said health plans should notify patients when they reach their lifetime or annual limits, but that doesn’t always happen.

Also note that while your insurance provider stays the same after you change jobs, your benefits can be very different.

Kudos to Bair for being a proactive patient and reaching out to the Attorney General – which got him a positive outcome.

OAKLAND, Calif. – While the Mariners weren’t at T-Mobile Park, outfielder Kyle Lewis, recovering from surgery to repair a torn meniscus in his right knee, has been busy working to get off the injured list and make a contribution to perform last month of the season.

“Kyle has gotten into baseball pretty well,” said Mariners manager Scott Servais. “He’s shagging balls in the outfield and raising his limb. Hopefully a rehab mission can start for him this weekend. This is the latest I heard. It’s not clear there, but that has been whirled around here lately as he feels a lot better. ”

The rehab stint started with a few games with a particular hitter and built him up in game form and played on the field.

“You sure want to get his legs under him and reduce his timing,” said Servais. “We also understand that we are here almost in September. So that the clock is ticking a little on September 1st and whether the players can come back and support and influence a club. We definitely want to make sure he’s perfectly healthy and feeling good about where his game is at before we consider activating him here in the Major League. He suffered tears in his anterior cruciate ligament and his inner and outer meniscus while playing with the plate just months after the Mariners turned him 11th. It would require reconstructive surgery.

Lewis would have another arthroscopic surgery ahead of the 2018 season to correct some problems in his knee. But after this procedure, Lewis stayed healthy and played without any significant health problems until this season.

Break-ins and fights

When A’s starter Paul Blackburn’s Jarred Kelenic sang into left field, it snapped a series of 24 consecutive record appearances without a hit. His last hit was a solo home run by Blue Jay’s reliever Trevor Richards on August 14th. During that hitless run, Kelenic struck nine times with just four walks.

“Jarred had a really good last homestand,” said Servais. “He didn’t have that much to do on this road trip and the teams are sure to make him a bit tougher. He doesn’t have many pitches to hit. Houston pitched us really well. You have a really good ability to run the game board. When he got pitches for a hit, he didn’t get it into play. ”

Kelenic isn’t the only one having problems. Outfielder Jake Fraley wasn’t the productive machine on the base before he was put on the COVID-19 injured list. He missed 15 games and wasn’t the same hitter.

He has had a .145 / .230 / .218 slash line in 19 games and 61 record appearances since his return. He has eight hits with a double, a homer, four RBI, six walks and 18 strikeouts.

“Jake played great and Jake’s calling card really was his ability to control the strike zone,” said Servais. “He just wasn’t chasing and worked his way through some hard punches and got a lot of big hits for us. No question.”

The 15 missed games at the IL and in the rehab stint afterwards had an effect.

“That really disrupted his season and certainly affected his timing a bit,” said Servais. “I think he’s showing some signs of getting out of there. And I feel good to have him out there. Jake’s entire game is really driven by his ability not to chase, control the hitting zone, and bring pitchers to him. ”

We all know that normal milk is devil juice. But what about non-dairy milk? Which one is superior? Well, I started drinking oat milk recently and I can’t believe I used to drink anything else. It is literally heaven and everyone needs to switch to this superior milk as soon as possible.

Look, I love the devil’s juice. If I could have it in my morning coffee every day, I would. But I named it after Satan for a reason, and that’s because my lower intestines feel like they are doing an exorcism when I drink it. Because of this, I’ve spent most of my adult life trying every non-dairy milk I could get my hands on.

I started with almond milk and see it’s ok. It was on everyone’s lips in 2015 and I wanted to feel like a middle-aged Caucasian woman. But unfortunately the taste is just not there. It’s not creamy enough like regular milk, and unless it’s a high-quality brand like Milk Lab – it literally tastes like an asshole.

Then I discovered soy and quickly became a convert. A soy latte was my favorite for years, followed by the underrated soy cappuccino. I really thought soy was my end game milk. I thought I found that one.

It foams better, goes well with espresso, and is better than almond. But there are some drawbacks. Soy milk is very easy to burn, and burnt soy milk is the equivalent of eating dirt. I would argue it’s even worse than a low brand almond milk. It is inedible.

What’s even more disgusting is that burnt soy milk can turn into that mysterious lump of soy. Imagine drinking coffee and then suddenly swallowing a strange lump. I hate it and feel emotional right now when I think about it.

Here too, soy milk is very brand dependent. I’m that annoying person who asks the barista what soy milk they use. I literally left a store because they use Vitasoy. Sorry, but I prefer to drink diarrhea.

Then one day I noticed that some of my friends were starting to order oat coffee and I was intrigued. It was hard enough for my pea brain to understand how almonds could be milked without an almond bite. But oats ?? Under no circumstance.

But I made it scorching hot and I really believe that I saw God. He was wearing a duck suit and sunglasses. Oh wait, when I think about it I think that was actually just Minor Figures’ oat milk logo.

What I love about oat milk is that it is creamy and has a really nice aftertaste. Look, it’s not milk. But I’ll take oat milk instead of cow’s milk and poop my pants every day of the week.

I am truly an oat milk convert and I URGENTLY encourage you to throw your almond latte or soy flat white in a bush or oncoming traffic somewhere (please don’t really do that) and drink oat milk instead. I have no other reason than that it tastes better and is kind of cooler.

Almonds and soy have a strange stigma attached to them. Maybe it’s because people associated this milk with the vegan movement five years ago? But oat milk is the new kid on the block and doesn’t bear that stigma.

Oat milk is superior to dairy-free milk, bitch.

LINCOLN, Neb. (KOLN) – A recent donation to the Autism Family Network is now helping families in Lincoln. The donation was made in honor of Damian Olander, a Lincoln man who suffered from autism and died of kidney failure last November. Damian’s family hoped that his life would teach others that sometimes we learn most from people who do not speak at all.

Damian Olander did not have an easy life. He was non-verbal, autistic, and struggled with kidney problems for 27 years. Damian was originally denied a kidney transplant because of his developmental delays. For the first few years of his life, Damian had to undergo reconstructive surgery every 6 months.

“It was an intense period of struggle for the first three or four years,” said his mother, Becci Olander.

The Olanders looked for transplant options, but Damian was turned down. Shortly afterwards, the Olanders learned that Damian’s father, Bob, was a match.

“It was never a thought,” said Bob. “I wanted to do it. It was my pleasure. I think any parent would have done it. “

Bob’s kidney gave Damian a second life that has kept him for almost 25 years to this day.

“You look at him every day and find that you kept him here,” said Bob. “We gave him this chance and he made the impact he made.”

Bob and Becci said Damian had an impact on everyone he met. He loved smileys, balloons, Halloween, M & Ms and most of all, spending time with his father.

“I would consider him my best friend alongside Becci. It has really been that way for the past 7 years, ”said Bob. “We hardly went without each other”

Two years ago, the Olanders learned that Damian’s kidney was failing. Instead of taking him through medical treatments, they decided to make the rest of his life fun. He was even allowed to spend his favorite vacation, Halloween, with the family.

“We had a big family party, everyone came in costume and in their favorite things, it was in the middle of summer,” said Becci. “We decided to make Halloween big for him this year. We had 80 jack-o’-lanterns in our front yard for Halloween. He wasn’t feeling well, pretty tired, but he loved that night. And it was the next weekend that he died. “

A few weeks ago, the family held a memorial event for Damian at the SaltDogs game. One family was so inspired by his story that they donated a thousand dollars to the Autism Family Network on Damian’s behalf. Becci said:

“He was non-verbal, but he spoke volumes.”

Becci and Bob said they were happy to know their son’s life was making a lasting impact. They suggest that you can also carry on Damian’s legacy by smiling patiently and enjoying the little things in life.
